Yes, if one is available please bring it in. If it is not available, please bring all the current medication bottles you may be taking.

Yes, you can also call your choice of pharmacy and request a refill through them. The pharmacy will then contact our office by phone or fax to request your refill.

You may go to any of the six clinics to request medical records. You may also access your records 24/7 on FollowMyHealth Portal. The patient must be present in order to sign the appropriate paperwork due to HIPAA regulations.
